DS-1150: Official Notice Of Personal Service For Suspension

If you have recently been charged with driving under the influence (DUI), you may be served with a DS-1150 Official Notice of Personal Service for Suspension by law enforcement. This form notifies you that your license has been suspended. After serving you with this form, the officer must take your license and send it to the Department of Driver Services (DDS).

Notification of Your License Suspension

The form states that you are being notified on this date that your driver’s license has been suspended, revoked, or cancelled. The reason for the action is listed on a line below this statement.

The DS-1150 goes on to state that it is unlawful for you to operate a motor vehicle in the State of Georgia. According to Georgia law, you may be charged with a misdemeanor if you fail to surrender your license to the DDS after it is suspended, revoked, or cancelled.

You must then surrender your driver’s license to the DDS or show why you cannot surrender it. If your license is lost or stolen, you must provide the DDS with a notarized affidavit (DS-250A).

The next section of the DS-1150 form contains fields for your personal information, including your full name, date of birth, address, sex, and license number. You must then sign the form, acknowledging that you have personally received service of your driver’s license suspension, revocation, or cancellation.

The bottom section is for the officer’s information. The officer must indicate if the license was picked up, and provide an explanation if the license was not confiscated. The officer must then sign and print the form, and fill out the remaining fields with his or her badge number, agency serving order, telephone number and agency’s mailing address.

The officer must attach your driver’s license to the form and send it to the DDS at P.O. Box 80447, Conyers, GA 30013.

Requesting a License Suspension Appeal

You have ten business days after receiving this form to request a hearing. In the request, you must state why the request is being made, the action desired, and the reason why you consider yourself entitled to have the action changed. The DDS will contact you to let you know if the hearing request has been approved or rejected. Send the request for a hearing to the address listed above.
